HIPAA-Compliant Form Processing — Jotform Gold with HIPAA-Enabled Features

All intake and contact forms on this Website are processed through Jotform (Gold plan with HIPAA-Enabled Features). Jotform's HIPAA-enabled plan includes AES 256-bit encryption at rest, TLS 1.2 encryption in transit, HIPAA-compliant data storage, audit trail logging, and access controls. Reign Recovery has executed a Business Associate Agreement (BAA) with Jotform as required under HIPAA, making Jotform a contractually obligated business associate responsible for protecting PHI in accordance with applicable law.

Standard Squarespace native form blocks are not used for health-related or admissions data collection. No form data is stored in Squarespace's servers or transmitted via unencrypted email notifications.

4. 42 CFR Part 2 — Substance Use Disorder Records

Heightened Federal Protection for Substance Use Disorder Records

Records relating to substance use disorder treatment are protected under federal regulations at 42 CFR Part 2. These regulations impose strict confidentiality requirements on any records maintained in connection with a federally assisted substance use disorder treatment program.

Under 42 CFR Part 2, Reign Recovery may not disclose your substance use disorder records without your written consent except in limited circumstances permitted by law, including a genuine medical emergency or a court order that complies with Part 2 requirements.

This protection applies even to disclosures to other healthcare providers, law enforcement, and family members unless you provide specific written consent for each individual disclosure.
