A provider-patient relationship is only established through a formal admission agreement signed by both parties following completion of the clinical intake process.

6. HIPAA and Health Information UPDATED
Reign Recovery is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and complies with all applicable HIPAA requirements. Our handling of protected health information (PHI) is governed by our Notice of Privacy Practices and our Privacy Policy.
Substance use disorder records are additionally protected under 42 CFR Part 2, which provides heightened confidentiality protections beyond standard HIPAA requirements.
All intake and contact forms on this Website are processed through Jotform (Gold plan with HIPAA-Enabled Features). Reign Recovery has executed a Business Associate Agreement (BAA) with Jotform. Information submitted through forms on this Website is encrypted, stored on HIPAA-compliant infrastructure, and handled in accordance with both HIPAA and 42 CFR Part 2.

13. Binding Arbitration

By using this Website and submitting any form (including ticking the mandatory consent checkbox on any intake or contact form), you agree that any dispute, claim, or controversy arising out of or relating to these Terms, the Website, or your use of the Website shall be resolved exclusively through binding individual arbitration rather than in court, except as set forth below.
Arbitration shall be conducted by a neutral arbitrator through the American Arbitration Association (AAA) or JAMS under their applicable rules, available at www.adr.org and www.jamsadr.com. The arbitration shall take place in Los Angeles County, California, or another mutually agreed location, and may be conducted by telephone, video conference, or written submission.
The arbitrator shall have the authority to award the same damages and relief as a court. The arbitrator's decision shall be final and binding and may be entered as a judgment in any court of competent jurisdiction.
This arbitration agreement does not apply to: (a) small claims court actions within jurisdictional limits; or (b) applications for injunctive or equitable relief related to intellectual property infringement.
14. Class Action Waiver

YOU AND REIGN RECOVERY AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR ITS INDIVIDUAL CAPACITY AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS, CONSOLIDATED, OR REPRESENTATIVE ACTION.
Unless both you and Reign Recovery agree otherwise, the arbitrator may not consolidate more than one person's claims and may not preside over any form of representative or class proceeding. If this specific paragraph is found unenforceable, then the entirety of the binding arbitration section shall be null and void.
15. Governing Law and Jurisdiction
These Terms and any disputes arising out of or relating to these Terms or the Website shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law provisions.
To the extent any matter is not subject to binding arbitration under Section 13, you consent to the exclusive jurisdiction of the state and federal courts located in Los Angeles County, California.
